Describing & Measuring Undergraduate Teaching Practices Registration Form

Registration for all sessions will be limited to 500 participants, on a first-come, first-served basis. Also, registration will be closed three days prior to the date of the session.

You should receive a confirmation email after you have completed registration. If you do not receive the confirmation email or have any questions, please email ygeorge@aaas.org.

Two to three days in advance of the Webinar, you will receive an email directing you to a link at which you will be asked to submit your name and email address. Once this information is submitted, you will immediately receive a set of logon instructions. Any questions concerning logon instructions and guidance materials can be directed to Roger Seals ceseal@lsu.edu or 225/335-5327.

All information you provide is confidential and will be used only for evaluation purposes by the collaborating organizations and the evaluator for the purpose of analyzing data, preparing reports and articles, and selecting samples for a limited number of carefully defined follow-up studies. Any information publicly released (such as statistical summaries) will be in a form that does not personally identify you or your institution. Your response is voluntary and failure to provide some or all of the requested information will not in any way adversely affect you.
